Please take the time to complete and submit an online Medical Questionnaire prior to attending the clinic for treatment.  This is available at :-

This should ideally be completed and submitted, even if you have previously submitted a medical questionnaire (apologies if you have already done so), at least 24 hours prior to your treatment. If you are not able to submit the questionnaire online prior to attending the clinic, we will ask you to do so on arrival. Unfortunately, we are only able to assess your suitability for safe treatment upon receipt of your medical questionnaire.

A deposit may be taken to secure your appointment, this will be deducted from the cost of your treatment, or refunded in the case of free consultation if you decide not to proceed with any treatment. In all other cases, the deposit is non-refundable for all bookings unless notice of cancellation is received at least 48 hours prior to the time and date of your appointment.

If you have paid a deposit via PayPal (you can also use debit / credit cards, you don’t need a Paypal account), your transaction has been completed. You can logon to your Sussex Laser Lipo account to check the details of your bookings, or logon to your PayPal account (if you have one) to check the details of your payment transaction.
